[BUG] The 'Required' checkbox disappears when the 'Multiple Choice' field is selected in the checklist section on the 'Create New Task Template' page.

Description

Environment: Staging.

Steps to Reproduce:

  1. Open the 'Create New Task Template' page.
  2. Scroll to the 'Checklist' section.
  3. Click the '+ Add Checklist Field' button.
  4. In the 'Field Type' dropdown, select 'Multiple Choice'.
